INSURANCE
It’s true! Most insurance plans pay for nutrition visits. However, insurance coverage is highly variable and ever-changing. Coverage often depends on what benefits you and your employer select, deductible amounts, specific diagnoses, and maximum yearly allowances for your contract.
Prior to your Initial Consultation, Simply Stated Nutrition will verify your benefits with your insurance company.

As a courtesy to you, Simply Stated Nutrition will submit a claim for nutrition services directly to your insurance company. All co-payments are due at the time of service. For your convenience, Simply Stated Nutrition accepts cash, check or credit card as payment.
Out-of-network: If Simply Stated Nutrition does not participate with your insurance company, you may choose to submit for reimbursement for nutrition services directly to your insurance company. Full payment will be collected at the time of your nutrition services. At the end of your appointment, you will be provided with a Super Bill (a receipt for nutrition services) to submit for reimbursement. For your convenience, Simply Stated Nutrition accepts cash, check or credit card as payment.
