Let’s get started. I will listen to your health concerns and support you on your journey to achieving your health goals. As we work together, I will help you to find and implement healthy eating patterns and lifestyle behavior changes that will empower you to achieve your goals.
I look forward to meeting with you in person in my office. In addition, Nutrition Services are also available via HIPPA compliant teleconferencing for all private pay clients. Availability of teleconferencing for those using insurance benefits for nutrition services is variable and depends on your specific insurance plan.
INITIAL CONSULTATION (CPT Code 97802)
Our first appointment will last about 60-90 minutes as we address your nutrition related health concerns, expectations and goals. We will also review and assess:
- your medical and nutrition history
- current food and nutrient intake
- wellness behaviors
- medical tests and lab results
- medications and use of nutritional/herbal supplements
We will also measure your height and weight (and possibly utilize additional indicators of nutritional status, such as measuring waist circumference and body composition) and calculate your BMI.
Taking this information into consideration, we will work together to set goals and create a Nutrition Care Plan. Nutrition education and supporting materials will be provided. You will have time to ask questions and get any clarification needed. You’ll also be provided with a Nutrition Care Plan summary to use between appointments and for us to monitor your progress.
FOLLOW UP SESSIONS (CPT Code 97803)
Achieving optimal nutritional intake is an ongoing journey. Follow up sessions will help you to keep on track and provide an assessment of your progress and the ongoing support needed to help you to meet your goals. Follow up sessions will last between 30 and 60 minutes, depending on what we establish in your initial Nutrition Care Plan. We will typically review any new medical information, your current food and nutrient intake, positive changes and barriers to change in wellness behaviors, changes in weight and BMI and assess areas of success and areas for improvement in your initial Nutrition Care Plan. From there, we will work together to update your Nutrition Care Plan. Pertinent educational materials will be provided. As always, you will have time to ask questions and receive any clarification needed.
