I am so pleased that you are taking the next step and scheduling an appointment.  Completing the following tasks prior to our first session will allow us to ensure that we spend the maximum amount of time possible on our most important task:  your Nutrition Care Plan.

Again, before your first appointment, please complete the following tasks before you arrive:

• Complete all online forms sent via the email messaging system.
• Check your email for appointment confirmation and other details.
• Review our location on the Google Map. Parking is available across the street from the Phillipsburg Education Center entrance.  An even closer, (wheelchair) accessible parking lot and entrance are available from the driveway on the left of the building if required.  Please call (908)-319-4339 if you need any further information on directions.
